Register for a Middle School Sport

What you must do to Participate in

South Milwaukee Middle School Sports

(Tackle Football, Cross Country, Wrestling, Basketball, Volleyball, Track)

 

Step #1 – Set up an account in Bound and pre-register your student/athlete.

During this process, you will need to upload the following documents:

  • A current physical form completed by a doctor.  The physical cannot be older than April 1st of the previous year.  ie:            2026-27 school year the date would be April 1st, 2025.
  • Photo of your current health insurance card (front side only)

If the sport you are pre-registering for is a “try-out” sport, this must be completed prior to the tryouts.  Failure to complete this step before tryouts will result in your student/athlete being unable to participate. 

If the sport you are pre-registering for is a “no-cut” sport, this must be completed prior to the first scheduled practice.  Failure to complete this step before the first practice will result in your student/athlete being unable to participate until it is completed.

Step #2 – Athletic Fee Payment:

The athletic fee payment is handled by the South Milwaukee Recreation Dept.  This must be done prior to the first practice.  The payment can be completed:
