Informational Letter

Dear Parents:

The following are instructions for your son/daughter participating in the Middle School Sports Programs offered thru the SM Recreation Department.  All fees & paperwork requirements are due at the time of registration.  If the program requires a physical, that can be submitted to the Rec Dept. by the first practice.

Parent/Athlete Concussion Agreement – one must completed for each school year.  This is signed by both the parent/guardian and the athlete.

Physical forms must be completed for each school year.  1st and 3rd years, the athlete must be seen by a doctor and have the Physical Form completed (Green form).  If the last physical is NOT older than 2 years, the doctor may be willing to use the last physical information to complete the physical form.  The 2nd year, the athlete may use the Alternate Year Physical Form (beige).  This needs to be completed by the parent/guardian and signed by both the parent/guardian and athlete.
